I'm afraid there are some incompatibility issues for Mellel 4.2 with mac os versions before Sierra (10.12), we are working on a fix and it will be available as soon as possible.

What's available in the insert table sheet depends on the contents of your style set. If you have only one table style with one template then that is what you're going to see. If you open a new document using the default Mellel template you should see the same styles presented in the video tutorial ...

The main difference is that PDFs produced by print->PDF don't include the outline.

Some things you might want to try and check 1. How did you produce the PDF? Did you use File->Export->PDF or did you use File->Print and then produced a PDF? Try using the opposite method and see if that makes a difference 2. Try producing a PDF from some other application (for example, Text Edit). ...

Regarding the issue with quick index not maintaining your last selection. Looks like a bug, we will fix that asap.
