As far as I can see, the only way to avoid this is to have live bibliography turned on from the very beginning. My testing is less exhaustive than the other posters above, but I have been having good resulting exporting to .doc in Mellel, importing that file in Nisus Writer Pro and then exporting from NWP to .docx. The results looks pretty good.

Addendum : Yes, that's the easiest way to go. I tested it. Now you have continuous page numbers, footnotes that restart with every new chapter, and every chapter starts on a new page. The only thing I can think of is, that you are nevertheless using two different note-styles. Not paragraph-style in the note, but note-attribute.
