My first impression is that the program concentrates too much, perhaps exclusively, on individual words instead of words in context. Therefore most of the suggestions were wrong. I have just installed the update 4.1.1 and then imported a file in .doc. All footnotes were lost as notes and appeared after the text without links.

Assuming your document has only one Page style, open the Page palette and click on the Attributes tab. Deselect "Start at:"
This should enable continuous numbering throughout the chapters.

Yes.

1. Open your Page style from the Page palette and tick the "Different first page" box in the lower left corner. Then
2. Insert "Page style break" from Insert -> Breaks after each chapter.

Is it currently possible to show comments in imported DOCX files? No. Only Word documents saved in Rich Text Format retain the comments. Documents saved as .doc show the comments as inline tex at the end of the document (not as comments though.) Tested with Word v16.11.1, Mellel 4.1. on macOS 10.12.6
