I think an image is currently your only option. An Illustrator file just containing the single capital letter would be a good choice.

I'm a little surprised that there's a live bibliography, but no live tables of contents. Live bib. requires a round-trip interaction with a piece of external software, whereas live contents would all be done within Mellel.

The issue occurs when you have paragraph line spacing set to a "lines" unit, and in the options of the image "adjust fixed line spacing to image" is selected. I think this is covered in either the Mellel guide or the tutorial.
