So the problem, as far as I can tell, has narrowed down to Mellel's export of the document. Put another way, the problem is narrowed down to Google Docs ;) (It's still 'beta' after all.) I took your advise and went through it again. They are all using the same paragraph styles, no defaults. Very odd behavior. I'm stumped too. Just tested it, no change. It is very odd. I figure it must be a paragraph setting, but it is sporadic - it is only effecting the first 10 or so items of the TOC (over 30 items).
