So the problem, as far as I can tell, has narrowed down to Mellel's export of the document. Put another way, the problem is narrowed down to Google Docs ;) (It's still 'beta' after all.) Just tested it, no change. It is very odd. I figure it must be a paragraph setting, but it is sporadic - it is only effecting the first 10 or so items of the TOC (over 30 items). Could the "Keep with next _ lines" setting in the Paragraph Style you use for the TOC be responsible - i.e. not set to "0"? Maybe several lines are set to keep with the next 1-2 lines, causing a cascade effect under certain circumstances?
