I might not be the greatest expert with Mellel but, for my experience, there is no way to preserve the list levels. The problem seems to have been with Bookends (although they told me it was with Mellel). Their latest release seems to have fixed it.

The bibliography works fine, it's the footnoted citations that don't.

I can confirm that effect. To be a little more precise, here in my document the italicisation of the book chapter moves three characters to the right and thus affects the data that follows and that should not be italicized. Formatting of journal articles citations is not getting to Mellel correctly. Note: I sent the problem to sonnysoftware support and was told it is a problem in Mellel. I can find the paragraphs without any problem – it's the replacing them with nothing part where I get confused. (and I am easily confused)
