When I click on paper size A4, I get a list of all the options and to the right of each is an arrow. Click on the arrow and the choices are A4 [default] and A4 borderless. Chris, I doubt if this is the answer, but just in case check out File/Page setup/Paper Size.
There are two definitions to "A4," regular A4 and borderless.
I you using the correct one for your work?

Curiouser and curiouser The only two places left where Mellel changes the look of printed out are in -- File/Page Setup [make sure you have 100% selected] and -- the Print/Mellel 4/Layout, but I don't know what you could do there to create the affect you describe. MonE, one thing that might cause this is annotations (comments)
-- if you select Export annotations [lower left of the Export to pdf window], and

then you will get exactly what you describe.

Of course, there's an obvious work-around: scan your signature into a jpeg file and insert that file in the text when you need it.

By the way, JBQ, I think you will find that 2017 is when the font file was stored on your system, but, if you check the info page for Hoefler Text in FontBook, you will see that the font was last updated in 2007.

JBQ, I can't repeat your problem with Arial or Times or any of the fonts I generally use,
but I definitely see it with Hoefler text.
Maybe this is an old version of the font that needs upgrading. The version I have is from 2007.

I've never actually felt the need to control the size of the cells per se -- I just let the character/paragraph attributes take control. On occasion I've used cell padding as well.
