Neither.

You can make an initial with Mellel, it's just a little messy: you have to play with indents and baseline shift.

This request has been raised frequently and is on the top of my "most wanted" list.

You can create a passable workaround by using tables. The one big drawback with tables is that a table cell can only be one page long.

If you want to make a booklet I would recommend to make your styles with bigger fonts and use CocoaBooklet to create the booklet you want. I do things even easier: I prepare my booklet on full-sized pages (with bigger fonts), make a pdf version and send it to the print shop to be printed in booklet...
