There are perl scripts to translate MD to OPML. I personally use MultiMarkdown, which includes additional features.

What about any of the Culmus fonts?

More on topic, the reason the diacritics would misalign is because of improper anchor points in the font itself. It may also have to do whether internally the fonts are TrueType or Postscript; OpenType fonts allow the fonts to be encoded as either.

Melle will export an outline (and therefore, Auto-Titles) to OPML. Does Mellel not import OPML?

Correct. Only applications purchased through Apple's Mac App Store can access iCloud Sync, because it needs to be signed by Apple. (At least, that was the case when it was unveiled. So it sounds like you notes are properly imported. Have you tried to apply a style to them through the Insert > Note > Edit note attributes… dialog? That should allow you to override all of your existing footnotes ...

Change your document's footnote attributes in the 'Edit style sets…', or use the 'Edit note attributes…' option.
