RTF is actually a Microsoft Word native format. The specs for the RTF format are updated each time Microsoft releases a new version of Word, and every feature that Word supports is actually part of the RTF spec. I suppose that seems to be a font issue, assuming you're using real small caps and not the fake type. If you're using real small caps, and are unable to use them in bold italic, are you certain that your chosen face offers a bold italic small cap?

No, that is not currently possible without some creative workarounds. You can rotate the image in Preview before pasting into Mellel. But for the other parts, there really isn't an easy way to achieve that.

To input the vowels while typing in Hebrew, your keyboard must support it not the app. If the default Hebrew keyboard for the iPad does not support nikud, there are a few third-party keyboards in the App Store that do.

Mellel does not show thumbnails. However, if you structure your documents using Auto-Titles, the Outline pane can be used for navigation and reordering.

Auto-Titles can be configured however you like. I personally do not care for the defaults; check out the Mellel Guide for an in depth explanation of Auto-Titles and their customization.

For interchange, your only real option is RTF.
