This is super easy.
1) download Ukelele (free)
2) File - New - from existing layout (pre Leopard) - OR get a starter layout from the disk image.
3) Double click on keys to change stuff. Hold down a modifier key to change modifier-key keys.
4) save it in Library/Keyboard Layouts. Make sure it has a keylayout extension.
5) Log out and log back in, then add the keyboard through sys prefs.
SUPER EASY
I created a keyboard that has option-` as kamatz katan, and I'm going to add ḥ to my English keyboard. These two things alone will add years to my life.
