I have a document in MLA format. Heading 2 is in-line and in italic.
How can I make this heading appear in the TOC, that is, assign a level to this heading so that it appears in the TOC?

To me it seems the only way to have a heading appear in TOC is to apply an auto-title in Mellel to a *whole paragraph*. Is this true?

For my understanding, auto-titles are the way to produce the Table of Content. The paragraph needs to follow on the following line.

Well, in fact you can do it manually. Just create the auto-title with the proper formatting and then add the remaining paragraph on the same line.
When you add an auto-title directly in a paragraph already written, all the paragraph will be automatically selected as an auto-title, even if you select only the first words.

I think you mean "and then format the remaining paragraph manually on the same line."
If I understand you correctly, I came to the same conclusion.
I managed to do it with a combination of a new Tag Flow and a paragraph style, which I applied to the rest of the paragraph.
