Keeping figure and table auto-titles above

Sometimes, a figure heading or a table heading (tag flows) will jump to be below the figure or table, rather than remain above it, as intended. What is causing this and how do I prevent it? This often happens if I delete what appears to be an extraneous carriage return between, say, the graphic and the next paragraph.
