Mellel 4.2 and Bookends are not working for me. I have latest versions of both. I cannot select Live Bibliography in Mellel. When I try to scan manually, Mellel gives the "Scanning Document" dialog, but Bookends never puts on the dialog to select a format. So everything stalls at "Scanning Document" -- only choice is to cancel. No bibliography is produced of course.

I've got it working. Not sure how, but I suspect it was a problem with giving my Mellel permission to control Bookends. I was sure I had done that. Perhaps not.

When running on Mojave you may need to enable Mellel and Bookends to work together (Apple taking yet another small step to their desired goal of making all this impossible)
Here's what you need to do:
1. Open System preferences
2. Click on Security & Privacy
3. Click on "Privacy"
4. Click on "Automation"
5. Locate Mellel in the list and make sure the checkbox next to bookends is checked
6. Do the same for Bookends (check the "Mellel" checkbox)

We've just released Mellel 4.2.2 which address this issue. For users with the direct version of Mellel choose Mellel->Check for updates, for users with the Mac App Store version, the new version is currently waiting for review at Apple, if all goes well and as usual it should be available in the next 24 hours.

Indeed, things seems to run smoothly with 4.2.2 : ) (haven't done extensive tests, but found no issues at all)
