Assuming your document has only one Page style, open the Page palette and click on the Attributes tab. Deselect "Start at:"
This should enable continuous numbering throughout the chapters.

Is it currently possible to show comments in imported DOCX files? No. Only Word documents saved in Rich Text Format retain the comments. This happens when you export to an external disk.

If you export to your Desktop and make sure all export options are enabled in the upcoming dialog, then the outline is exported correctly. No need to additionally create a TOC.
