I’ve never had this sort of trouble with Mellel myself, so I’m speculating. If iCloud is having a hard time keeping up with your edits of such a large file, the problem might go away if you did your editing on a local copy of your document that isn’t being backed up to iCloud. Different reference managers handle in-text citations differently. Are you using Bookends to insert temporary pale blue in-text citations that are then scanned? This may not clear up all your mysteries, but Page Setup gives sizes in inches regardless of Mellel's preference setting -- that particular box gets drawn by the OS rather than by Mellel. I always have my Mellel pref set to Pts but Page Setup in particular always shows me inches.

There are a lot of details concerning how footnotes and endnotes are marked and managed that you can adjust via Insert > Note > Edit Note Attributes.
