I only have
Title
Heading 1
Heading Two
etc.
Where is the "Chapter" Autotitle level hiding? Do I have to create it myself?

What your screen shot shows is the default Auto-title configuration provided for by Mellel’s blank templates. The illustration of the Configure Auto-titles window in the User Manual shows a document with a different custom configuration, where one top level stream has been given the identification “Chapter.”
That window, reached by choosing Insert > Auto-title > Configure Auto-Titles, allows you to name and arrange Auto-Title streams as you see fit. To name an existing stream “Chapter”
Select the Streams panel of Configure Auto-Titles
In the left hand list, select the stream you want to designate for chapters.
Then hit the oval “Change” button after Identification, to the right of the big screw icon.
